SWCHCC Grant Funding Process

This page provides information to the SWCHCC and its members regarding the process of applying for grant funding. Agencies that apply for funding from the SWCHCC must be located within the jurisdictions served by the SWCHCC: Archuleta, Dolores, La Plata, Montezuma, San Juan, Southern Ute Indian Tribe and Ute Mountain Ute Indian Tribe.

The SWCHCC Steering Committee will review and score projects based on the established criteria. Since the grant funding available is provided by the Hospital Preparedness Program (HPP), the SWCHCC must adhere to specific requirements from both the Assistant Secretary for Preparedness and Response (ASPR) as well as the Colorado Department of Public Health and Environment (CDPHE).
